General Information/ Policies
- If you are a new patient, please download, print and fill out the Initial Intake and Consent Forms located at the bottom of this page and bring them with you to your first appointment.
- If possible, please wear loose, comfortable clothing to your treatment sessions.
- If you need to reschedule or cancel an appointment, please do so at least 24 hours prior.
- Payment can be made in cash or check. Insurance is not accepted at this time.
- Please be on time for your scheduled appointment, or call if you are going to be late.
